If you have Medicaid and need assistance with food, housing, education, employment, transportation, medical care, and more — we may be able to help.

Through the Social Care Network (SCN), CCHP connects patients to the social services they need most.

Do I qualify?

To qualify for SCN services, you must:

  • Have active Medicaid

  • Live in Manhattan, Brooklyn, or Queens (Bronx residents: check out Somos, a similar program near you!)

  • Complete a screening with one of our patient navigators

How screenings work

CCHP's patient navigators can meet with you to see if you qualify. Screenings are available:

  • In person at CCHP (walk-in or by appointment)

  • By phone or Zoom

  • In the community — at local events and gatherings

What you can get connected to

If you qualify, we can connect you to services for:

  • Food

  • Housing

  • Education

  • Job connections and training

  • Transportation

  • And more
